Then the condition that would reverse it: a number, a date, a threshold. A decision with no reversal condition is a belief. And it says whether the decision is reversible, because most are and get treated as though they are not.

No. Where there is a genuine fork it names both, recommends one, and says what would change its mind. A memo that only presents options is a briefing.
Why record the dissent?
Because it costs nothing and it is what makes people willing to disagree next time. It also notes when a decision was unanimous, which is a signal and usually not a good one.
What is the reversal condition?
A named threshold, number or date that would make you revisit. Without one the memo records a belief rather than a decision.
How long should a decision memo be?
